Prior to submitting your request for facilities use, please review Board policy and regulation 1330, Use of Facilities (Policy & Regulation), including references to policies 1230, School Connected Organizations, 3514 Equipment, 3515 Smoking Prohibition; and, 6145 Extracurricular Activities.

I/We have read and agree to the terms as set forth in the West Morris Regional High School District's Board Policies and Regulations.
I/We understand that by completing the Use of Facilities form there is no guarantee the request will be approved.
